The Partner Assessment

Unlike a traditional request for information (RFI), potential partners are being asked to complete a Partner Assessment consisting of two parts designed to determine fit and philosophical alignment: an Interest Survey and a Habit Ability Assessment.

Interest Survey

This section is comprised of 20 questions and will take approximately 20 minutes to complete. Before you begin, you should be prepared to answer questions relating to the following topics:

  • Logistics of contracting and implementation
  • Back-end features and functionality
  • Relevant regulatory requirements

Habit Ability Assessment

This section is comprised of 80 questions and will take approximately 1 hour and 40 minutes to complete. Before you begin, you should be prepared to answer questions relating to the following topics: 

  • Design of product or service
  • How it works to create habit change
  • Specific examples of the offering's effectiveness in an employer setting

The desired outcome is to contract with select, high quality strategic partners who will support the execution and measurement of our clients’ employee performance strategies, and who are committed to participating in co-marketing efforts.

DEADLINE FOR SUBMISSION: Midnight July 15, 2016 (Central Standard Time)


Who is Habits at Work?

Habits at Work is a consulting company that supports employees to create pivotal habits that lead to their health, happiness, security and high performance at work, leading to flourishing companies.

We help companies become employers of choice by:

  • Helping them focus on the pivotal habits that are really worth doing
  • Teaching them how best to create those habits over time
  • Evaluating the actuarial and financial impact of inaction or success

This evaluation is also being completed as a preliminary step towards a final contracting phase on behalf of Premera Blue Cross Blue Shield and Vivacity, an independent company providing wellness services.

Premera Blue Cross Blue Shield insures or administers health insurance benefits on behalf of 2 million members in Washington and Alaska, including 400 employers with more than 200 members, and several of about 10,000 members.

Vivacity provides wellness services to many of those members and employers, as well as to some employers that are clients of other carriers.


Please note: The initial analysis performed by Habits at Work will act as a pre-qualifier for consideration by Vivacity. Additional screening, reviews and evaluation may be conducted directly by Vivacity. Habits at Work is NOT acting as an agent of Premera Blue Cross Blue Shield in this initial phase of the evaluation. Habits at Work may contract with those partners who we choose, and we MAY work with Premera Blue Cross Blue Shield on securing a partnership with them. Habits at Work makes NO representations, promises (implied or otherwise) about the final actions that will be taken by Premera Blue Cross Blue Shield.
